Victoria: Submit via the Department of Health FOI unit at health.vic.gov.au or directly to AWH. The application fee is $29.90. You have the right to request documents relating to policy and planning decisions.

NSW: Submit via the Government Information (Public Access) Act 2009. No fee for informal requests. Formal GIPA applications: $30.

Commonwealth: Submit via the Department of Health and Aged Care FOI unit under the Freedom of Information Act 1982.
